| All instructions and information necessary to apply for certification can be found below. Please do not write asking for an "application" or "information packet". If you write asking for additional information, please be very specific. |
| 1. The certification process is initiated by the certification candidate and begins with an application which consists of: |
| 1a. A cover letter addressed to Management Science Institute stating, specifically, the certification for which you are applying. The letter should also state that all the information provided is true and accurate. The letter must contain your full legal name and address. |
| 1b. One set of school transcripts. Include transcripts for all degrees earned (or in process), and any applicable continuing education. |
| 1c. Two professional letters of reference |
| 1d. Two personal letters of reference |
| 1e. A resume of your educational and professional experience |
| 1f. A copy of your birth certificate |
| 1g. A copy of a photographic identification |
| 1h. One copy each of your thesis, dissertation, or any published abstracts or articles |
| 2. The eight items listed above comprise your application for certification.
There are no additional forms to complete. The cover letter must be notarized. Place all the above items in an envelope in the
order listed above* along with $295 (US)**
in the form of a certified check or money order for
each certification application. Mail the complete application to:

| 3. All information pertaining to certifications is on this Web site. If you have questions after you have reviewed the certification requirements, you may contact Management Science Institute and your questions will be answered. | 4. Once your application has been received, it will be reviewed
by MSI. MSI will either approve or reject the application
or they may ask for additional information.
Once the application has been accepted, you will be notified of your examination requirements. Examinations may include any combination of essay, multiple choice, or true/false questions. Examinations are tailored to the individual candidate. Examinations have no time limit to complete but you should allow 2-4 hours for completion. Examinations must be scheduled to be taken within 185 calendar days of the application's acceptance. MSI reserves the right to waive the examination. The candidate is responsible for scheduling a proctored examination. Most colleges and universities, and many employers, have testing centers which provide proctored examination services. The candidate is responsible for any fees charged by the testing center. The test will be mailed directly to the testing center and the testing center will return the test to MSI upon completion. MSI will pay for return postage of the test. Approval or denial of the certification is at the sole discretion of the Management Science Institute. Please review qualification requirements by selecting a certification from the list above. |
| 5. Management Science Institute's certifications are available only to MSI members. There are no membership fees or dues for an associate level membership. You may join MSI by completing the on-line membership application. |
| 6. If your certification application is not approved, your application fee, minus $75 (US), will be returned to you. If the candidate does not complete the certification process for any reason other than MSI's rejection of the application, the candidate's application fee will not be refunded. |
